Community Homes

Elmwood presently oversees eleven Community Homes, with an additional four slated to become operational by the conclusion of 2024 through our Manor Transition Initiative, thus increasing our total to 15 homes. In each of these homes, we offer outstanding person-centered support within a caring family environment.

Being a resident of a Community Home supports individuals to foster life-long friendships. Support is personalized to meet each individual’s unique needs and foster self-determination, respect and continuous growth.

Moreover, Community Homes promote community inclusion and belonging through a vast number of recreational and social activities.

Our Resident First Approach also ensures that every aspect of the individual’s needs is constantly being explored and met – including emotional, physical and spiritual needs.

  • All individuals considered for Elmwood’s Community Homes must meet the mandate of the Ministry of Social Services – Community Living Service Delivery.
  • Applications are submitted through the individual’s Community Services Worker (CSW) with the
    Ministry of Social Services – Community Living Services Delivery (CLSD).
  • Applications considered for Community Home placement must include a Saskatchewan Association of
    Rehabilitation Centres (SARC) Application and a recent Daily Living Support Assessment (DLSA)
    cover sheet.
